Client Disclosure Authorization Print E-mail
Effective January 1, 2009, before we can present ANY of your information to outside parties for any reason, we must have your WRITTEN authorization in our files per Internal Revenue Code § 301.7216-3.

This authorization is generic in that it covers Entities (Businesses) and Individuals.  If the tax return was prepared with Married Filing Joint, both spouses must sign the authorization.

You may fax or scan and email the completed form.  Again, we MUST have this in our files FIRST.
